Hospice Medical Director Physician

The Hospice Medical Director Physician Designee (“Physician Designee”) assumes the responsibilities of the Hospice Medical Director in their absence, in accordance with the mission and goals of the Elliot Health System (“EHS”), VNA Home Health and Hospice Services Inc. (“VNA”) and the objectives established by the Vice President, Home and Community Services , The Physician Designee is the role model of appropriate, expert, compassionate, and cost effective clinical care and maintains exemplary inter-personnel relationships with colleagues in the specialty group and throughout the Organization. He/she fully embodies and upholds the Elliot Hospital Code of Conduct. Provides care in a professional, efficient and ethical manner, as well as in accordance with the Hospice Conditions of Participation, and State of NH Hospice licensing rules, applicable laws and regulations of the State of New Hampshire, and philosophy of EHS.

JOB RESPONSIBILITIES:

  1. Actively participates as a member of the interdisciplinary group (IDG), acting as the medical resource, and providing guidance to other IDT Members. Reviews medical care provided in patients’ homes and other health care facilities where hospice patients may be located.
  2. Consults with attending physicians as needed regarding pain and symptom management. Maintains liaison with the hospice patient’s attending physician and encourages the attending physician to provide primary care to their patient in collaboration with the hospice interdisciplinary team.
  3. 3.Provides consultation with attending physicians, as requested, regarding pain and symptom management.
  4. Effectively coordinates efforts with the attending physician to provide medical care in the event the attending physician is unable to retain responsibility for patient care.
  5. 5.Reviews patient clinical information and eligibility for the Hospice Program in accordance with policies and guidelines established by the Hospice Program and Federal and State regulations. Certifies and reviews on an ongoing basis patient medical eligibility for Hospice services in accordance with program policy. Completes initial and subsequent certifications regarding terminal illness in a timely manner,
  6. Establishes a comprehensive, written plan of care for each patient collaboratively with the attending physician. Reviews and updates each plan of care on an ongoing basis.
  7. Provides leadership and dependable clinical guidance to Hospice staff. Responsible for supervising Hospice Physicians.
  8. Makes home visits to hospice patients on an as needed basis
  9. Raises awareness regarding hospice services in the community and health system.
  10. Assumes physician on call coverage for hospice ensuring coverage 7 days per week/24 hours day in the absence of the Hospice Medical Director
  11. Performs additional duties and responsibilities as deemed appropriate by the Vice President, Home and Community Services.

KNOWLEDGE, SKILLS AND ABILITIES Individuals must possess the knowledge, skills and abilities listed or be able to explain and demonstrate that the individual can perform the essential functions of the job, with or without reasonable accommodation, using some other combination of skills and abilities and to possess the necessary physical requirements, with or without the aid of mechanical devices to safely perform the essential functions of the job:

  1. Physical requirements include the ability to extend hand(s) and arm(s) in any direction; raise objects from a lower to a higher position or move objects horizontally from position to position; pick, pinch, type or otherwise work with fingers rather than the whole hand or arm as in handling.
  2. Ability to express or exchange ideas by means of the spoken word.
  3. Ability to exert up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects.
  4. Visual acuity sufficient for work which deals largely with preparing and analyzing data and figures, computer terminal operations, extensive reading, writing and telephone usage.
  5. The ability to plan and perform complex work, which involves new or constantly changing problems where there is little accepted method or procedure. Involves participation in the formulation and carrying out of policies, objectives and programs for major divisions or functions. Considerable ingenuity and exceptions judgment required to deal with factors not easily evaluated, interpret results and make decisions carrying a great deal of responsibility. Direct and coordinate the work of subordinate supervision in order to attain objectives.
  6. Ability to transport self to and from off-site locations.
  7. Competency with end of life care and pain and symptom management
  8. Strong communication skills
  9. Ability to deliver patient care in a manner that is appropriate to patient’s age, physical ability and intellectual capacity and that is culturally sensitive.

OSHA RATING: 1

WORK SCHEDULE:

Generally Monday through Friday, days. On-call, evening, night and weekend scheduled and unscheduled work time. Periodic overnight travel for meetings and educational programs may be required.

QUALIFICATIONS:

Medical degree from an accredited medical school Clinical and administrative management experience preferred. Licensed to practice medicine in the State of New Hampshire. Certification in hospice/palliative care preferred Medical degree from an accredited medical school Clinical and administrative management experience preferred. Licensed to practice medicine in the State of New Hampshire. Certification in hospice/palliative care preferred
